Tuition Assistance Checklist


Application for Tuition Assistance

When member applies for Tuition Assistance (TA) funding, follow the TA Check-List:

  1. Consult with the unit Education Services Officer (ESO) to locate the programs and courses of interest, and for assistance in completing the application for Tuition Assistance (CG Form-4147).
  2. Ensure the Institution is properly accredited. Regional/National accreditation should be stated in the Institution's catalog.
  3. Ensure service obligations requirements are met as outlined in TA Eligibility and Limitations.
  4. Complete the application form CG-4147 with ample lead-time for approval by the ESO and command . TA application must be received by the CG Institute at least 14 days and not more than 3 months prior to course start date.
  5. Note the following when completing the CG-4147 form:

    • Block 4 - Include dashes in this format: YYYY-MM-DD (when completing online)
    • Block 7 - Indicate "YES" if this is the member's last required course to receive a degree
    • Block 9 - Indicate education goal
    • Block 11 - must be completely filled in. "Unit Hours" : indicate the number of hours of each course. Also enter "S" = Semester hours or "Q" = Quarter hours. "Tuition Costs": enter only the tuition cost per hour in "Per Unit" column. No fees should be included in this cost.
    • Block 12 - Include work and fax phone numbers, signature and date signed
    • Block 13 - Provide the CO's, ESO's, or designee's name, email address, unit name, work and fax phone numbers

  6. Carefully read the Statement of Agreement/Understanding on Page 2 of CG-4147, and sign the form. Page 2 defines the Statement of Agreement/Understanding in which the member agrees to follow the established guidelines, and to repay the government share of tuition assistance if the course is not satisfactorily completed.
  7. Deliver the completed CG-4147 to the ESO for Command Certification & signature.
  8. Retain a copy of the CG-4147 for personal records. Follow-up if the TA Authorization form (CGI-1560) is not received within 14 days of faxing the CG-4147 to the CG Institute. CG Institute processing time may take up to 14 days during heavy registration periods of the year. The ESO should contact the CG Institute by calling the TA Hot line (888) 532-8257 or (405) 954-1360.

Note: In situations where a member is unable to submit their TA application prior to the course registration/start dates due to extenuating circumstances; the member may apply for waiver. Waiver submissions are discussed in How to Request a Waiver for Tuition Assistance.

Registration for Course

  1. Sign the TA Authorization form (CG-1560).
  2. If you are a reservist, provide your complete SSN in the top left corner of your authorization.
  3. Make a copy of the form for personal records.
  4. Take the CG-1560 to the college on day of registration.

Submission of Grade Report

ESO Action

  1. Review the CG-4147 closely. Ensure it is properly completed prior to accepting the form.
  2. Ensure member does not have separation orders on file or is being processed for separation, which would result in not meeting the service obligation requirements outlined in TA Eligibility and Limitations.
  3. Note the following when completing the form:

    • Block 6 - Include the school code from the CG TA School Code List. If the school is not on the list, contact the school for a copy of their "Accreditation Certificate" and send it to CG Institute with the school's address and point of contact.
    • Block 13 - Sign the form if so designated by the Commanding Officer, or forward to the appropriate official for signature. Include all information requested in Block 13. Ensure the name and title of the authorizing official is legible.

  4. Email the completed CG-4147 to TuitionAssistance or fax to the CG Institute (405) 954-7247 at least 14 days and not more than 3 months prior to course start date. Please allow 14 days for processing at CG Institute and delivery of the CGI-1560 to the member.
  5. Retain a copy of the submitted CG-4147 and approved CG-1560 forms at the unit for two years in compliance with MLC inspection requirements.
