6 — Safe Work Authorizations

Modified several sections to:

1. Reflect that the Job Hazards Analysis process replaces the Job Hazards Questionnaire (including adding in language on phase-in of the JHA), and that JHA replaces Division ISM Plan as the primary tool for Line Management Authorization
2. Change “Work Leader” to “Work Lead”
3. Remove references to HEAR system in Project Planning

6, Appendix B — Formal Authorizations

Revised to add chemicals that are secondary explosives, such as triaminotrinitrobenzene (TATB), and cyclotrimethylene-trinitramine (RDX).

6.4 — Safe Work Authorizations

Section rewritten to reflect recent changes in Chapter 21 of the PUB-3000, and to clarify that building safety analyses performed during the capital asset acquisition process are not reviewed after initial occupancy.

Revised section to meet requirements of DOE-BSO Report, “Employee Concerns and Whistle Blower Protection Programs Assessment," and to establish a process for informing personnel occupying the buildings after the initial briefing.

25 — Machine Safeguarding: Shop and Lab Machine Safety

Formerly called "Shop Safety and Power Tools." Major revision to chapter.

31 — Non-Construction Safety Assurance for Subcontractors, Vendors, and Guests at LBNL Facilities

New chapter.

31, Appendix C — JHA Form

Change title of form from "Job Hazards Analysis Form for Non-Construction ..." to "Job Hazards Analysis and Work Authorization Form for Non-Construction ..."  Several callouts in the text of Chapter 31 will also need to be revised to reflect this change in the title of the form.

On the green signature block (for Requester/Division Approval): Change the block to read from the following “"Requester/Division Approval:” to “Requester/Division Approval:  I authorize the work to proceed subject to the specified controls."

These changes are required in order for the Chapter 31 work authorization process to achieve equivalence with the institutional JHA requirements in PUB-3000 Chapter 32.

32 — Job Hazards Analysis

Updated chapter based upon lessons learned during the JHA Pilot Program. There are no substantive policy issues raised in this update. Changes include:

• Reworded implementation schedule by removing references to FY 2007 which has passed.
• Clarified that Worker’s signature, although desired, is not required. Work Lead’s signature verifies that the JHA has been discussed with the Worker, that readiness review has been conducted, and that the Work is authorized.
• Pulled out Appendix C as separate, downloadable document; revised the signature blocks on these documents to reflect implications noted above re: Worker and Work Lead signature.
• Clarified that JHA requirements for construction are given in PUB-3K, Chapter 10.
• Refined descriptions of typical tasks, hazards and controls as used in the JHA.

Clarified that the Individual Baseline JHA is accessed through the automated JHQ software, and that the Task-based JHA is a paper system not accessed through a central system.

32 — Job Hazards Analysis

Major revision to chapter and new appendices to reflect changes to JHA process.
