1. Engaging Leadership

The Chronic Care Model not only improves patient care and prepares residents to deliver health care more efficiently and effectively, it can also enhance a hospital's bottom line.

Hospital administrators are faced with many challenges and may not initially have the resources to focus on ambulatory chronic care. Engaging these leaders in the Chronic Care Model approach is important for successfully transforming the health care system.

For example, the University of Cincinnati Academic Health Center and Vanderbilt University Medical Center each emphasized the bottom-line benefits of the Chronic Care Model to build a convincing business case for dedicating time and resources to adapting the model.
