a Percents are age adjusted to the 2000 standard population using 3 age groups: 18-39, 40-59, 60 and over. Hypertension is defined as either having elevated blood pressure (average systolic pressure of at least 140 mmHg or average diastolic pressure of at least 90 mmHg) or taking antihypertension medication. Blood pressure under control is defined as average systolic blood pressure less than 140 mmHg and average diastolic blood pressure less than 90 mmHg based on average of 3 measurements and taking antihypertension medication.

b "Negative or poor" refers to household incomes below the Federal poverty line; "near poor/low income", over the poverty line to just below 200 percent of the poverty line; "middle income", 200 percent to just below 400 percent of the poverty line; and "high income", 400 percent of the poverty line and over.
