Application Instructions

How to Request a State Plan Extension

State Plans are normally approved for up to four years, but amendments to the original plan can be submitted for two years or one year.

State plan extensions should be submitted to the appropriate AoA Regional Office for initial review.

At the end of the four-year period however, a new plan must be submitted since there is no statutory authority to extend beyond a four-year period.

The final determination of approval of the initial plan or amendment is made by the Assistant Secretary for Aging.
