Sonat Exploration Co. Catastrophic Vessel Overpressurization

Summary
The CSB investigated a catastrophic vessel failure and fire near Pitkin, LA, at the Temple 22-1 Common Point Separation Facility owned by Sonat Exploration Co. The investigation found that Sonat management systems for hazard analysis during the design and building of the facility were inadequate. In addition, the investigation found that Sonat engineering specifications did not ensure adequate pressure relief protection for equipment that could potentially be exposed to high pressure hazards. Furthermore, Sonat management did not provide operating procedures for the start-up and operation of the facility. The CSB issued safety recommendations based on these root and contributing causes.
