The CSB investigated a toxic hydrogen sulfide gas leak at the Georgia Pacific Naheola mill in Pennington, AL. Investigators discovered a failure to follow good engineering and process safety practices and no management system existed to incorporate hazard warnings about sodium hydrosulfide into process safety information. Contributing causes included a failure to ensure the sewer remained closed and inadequate training for contractors about the hazards of hydrogen sulfide. The CSB issued safety recommendations based on these root and contributing causes.
