WITHHOLDING/CONTRIBUTION RATE ERROR

There is a misprint in Exhibit A of Payroll Office Letter 99-24 regarding the monthly withholding and contribution rates for the HIP Health Plan of Florida -- enrollment codes 3N1, 3N2, K71 and K72. The correct the monthly withholding and contribution rates for these plans are, as follows:

ADDRESSES FOR HEALTH BENEFITS PLANS (EXHIBIT B)

Exhibit B of Payroll Office Letter 99-24 contains the most current addresses we have on file for the plans that are participating in the Program for the year 2000. However, due to printing and formatting errors, Exhibit B is incomplete. Enclosed with this letter is a listing of the plans and addresses that were omitted. Please be advised that these addresses are official and should be used to mail all enrollment documents and enrollment reconciliation-related information.
