    Cruise Ship's Corrective Action Statement

Item No.: 02
Site: Medical-GISS Logs
Violation: During the voyage ending on 11/04/2008, one passenger case was listed as a pharmacy sale (Imodium) , listed as not reportable, even though the number of loose stool was listed as "many". A review of the medical records indicated the case-patient had his colon removed in 1992.
Recommendation: Ensure the GIS log lists underlying medical conditions for case-patients who meet the operational case definition but are listed as non-reportable.

Site: Potable Water-Bunkering Records
Violation: The analyzer chart recorded a free halogen value below 2.00 ppm for the three hour bunkering period on 11/11/2008. Bunkering began at 0900 and a manual check was done at 1015. The manual halogen check indicated a value of 2.15 ppm while the chart value was 1.8 ppm.
Recommendation: Ensure that potable water is continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device that is controlled by a flow meter.

Site: Potable Water-Engine Room
Violation: A transfer line was located in the engine room that allows transfer of potable water from the potable water storage tanks to the technical water tanks or the grey water tank. Staff stated that a hose was attached to the discharge line and ran over to the access hatch of the grey water tank for transferring.
Recommendation: The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at the following locations, if present: (1) Potable water supply lines to swimming pools, whirlpool spas, hot tubs, bathtubs, showers, and similar facilities; (2) Photographic laboratory developing machines and utility sinks; (3) Beauty and barber shop spray-rinse hoses; (4) Potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets such as those serving tanks containing chlorine and other chemicals, and deck taps; (5) Garbage grinders and pulpers; (6) Mechanical warewashing machines; (7) Hospital and laundry equipment; (8) Air conditioning expansion tanks; (9) Boiler feed water tanks; (10) Fire systems; (11) Toilets; (12) Potable water, bilge, and sanitary pumps that require priming; (13) Freshwater or saltwater ballast systems; (14) Bilge or other waste water locations; (15) International shore connection; and (16) Any other connection between potable and non-potable water systems.

Site: Potable Water-Cross-connection Control Program
Violation: The engine department listing of cross-connection control devices/methods did not include the mineralizer drain line air gaps and the atmospheric vacuum breaker devices on the potable water tank sampling taps.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Violation: One of the side suction fitting covers had a missing fastener.

Violation: The depth marker lettering was too small to be seen from in the pool.
Recommendation: Ensure depth markers are visible from all sides of the swimming pool.

Site: Whirlpool/Spa-Shock Halogenation Records
Violation: The free halogenation level was checked at the beginning of the shock halogenation cycle, but not at the conclusion of the cycle to ensure that the level was maintained.
Recommendation: Ensure that the free residual halogen is increased to at least 10.0 mg/L (ppm) in whirlpool spas and circulated for at least 1 hour at the end of each day.

Site: Whirlpool/Spa-Port and Starboard Mid-ship Whirlpools
Violation: The side suction fitting covers did not meet the AMSE/ANSI A112.19.8M or equivalent standard.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Site: Provisions-Fruit Walk-In Refrigerator
Violation: Orange peels were noted inside a container with unpeeled fruit, indicating consumption of fruit inside the unit.
Recommendation: Ensure that food is protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Violation: A bag of flour was torn exposing some of the contents to potential contamination.
Recommendation: Ensure that food is protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Violation: Slotted fasteners were found in the water contact zone of the ice machine.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: Slotted fasteners were noted on the interior of the ice machines. The slotted fasteners in ice machine #4.04A were badly corroded.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.
