    Cruise Ship's Corrective Action Statement

Item No.: 02
Site: Medical-
Violation: There were no shipping containers for mailing diagnostic specimens.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff.

Site: Buffet-Forward Beverage Station
Violation: A non-continuous pressure-type backflow prevention device was installed on the potable water line supplying the ice machine.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Medical-
Violation: The close contacts of crew members ill with gastrointestinal illness were only interviewed initially. There was no record of a 24 or 48 hour interview and the nurse stated that the contacts were only told to call if they get sick.
Recommendation: Ensure that medical or supervisory staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms and that these interviews are documented.

Violation: The close contacts of crew members ill with gastrointestinal illness were only interviewed initially. There was no record of a 24 or 48 hour interview and the nurse stated that the contacts were only told to call if they get sick.
Recommendation: Ensure that medical or supervisory staff conduct verbal interviews with asymptomatic cabin mates or immediate contacts to confirm their condition, advise them of the hygiene and handwashing facts, and instruct them to report immediately to medical if they develop illness symptoms.

Violation: On 10/6/08, there was a food employee ill with gastrointestinal illness. The last symptom was recorded as diarrhea on 10/7/08 at 1100 and the documentation indicated that the food employee was released from isolation on 10/8/08 at 1700.
Recommendation: Ensure that symptomatic food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 48 hours.

Site: Other-East Meets West
Violation: The time control temperature log consistently showed 100 °F as the starting temperature for sushi rice. Most times this rice was used for more than 3 hours. The chef explained that the cooked rice was removed from cold storage and mixed with hot vinegar then placed on the line for making sushi. The cooling logs for the cooked rice were in order.
Recommendation: Ensure that the time control log entry for the sushi rice begins when it is removed from refrigerated storage.

Site: Bar-Crew and Passenger Buffets
Violation: Side shields were not provided at the sneeze guards in several locations.
Recommendation: Install side shields on sneeze guards in locations where food is placed on display within 1 meter of customers lining up for service.

Violation: The handle on the right door of the clean utensil/plate storage cabinet was missing, resulting in a rough surface that was difficult to clean.
Recommendation: Replace the handle.

Violation: A counter profile strip was missing to the right of the right dipper well exposing deteriorated particle board material.
Recommendation: Replace the profile strip.

Site: Galley-Beverage Stations
Violation: There were raw wood surfaces along the door magnet latches for the doors. There were exposed sections of raw wood along the inside section of the louvered doors. Several of the doors had framing that was coming loose. All of these conditions made cleaning difficult.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Other-Bistro Dishwash
Violation: The data plate for the dishwash machine did not include the conveyor speed of the machine.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Site: Other-Bistro Potwash
Violation: The data plate for the potwash machine did not include the cycle times for the machine.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Site: Other-Deck 5 Four Seasons Dining Room Starboard Storage Room
Violation: The bottom storage shelving was too close to the deck. Items stored on these shelves were less than six inches from the deck.
Recommendation: Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 150 mm (6 inches) above the deck.

Site: Food Service General-
Violation: There were several "wash hands often" signs missing from the handwash stations in the East Meets West, and Bistro galleys. This was corrected during the inspection.
Recommendation: Ensure that a sign stating "WASH HANDS OFTEN" in a language that the food employees understand is posted over handwashing sinks.

Site: Galley-Garden Cafe-Toilet Room
Violation: There was no paper towel dispenser or paper towels. There was an electric hand drying device in this toilet room.
Recommendation: Ensure that galley toilet rooms have a paper towel dispenser stocked with paper towels available at the handwashing sink.

Site: Bar-Champagne Bar
Violation: There was an unsealed deck penetration in the technical compartment under the speed rail.
Recommendation: Seal the deck penetration.

Violation: The deck drain under the ice well was soiled.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Violation: There was excess water in the void space for the grill fire doors. There were wood strips in this void space that were water damaged.
Recommendation: Remove the water damaged wood strips and ensure this void space is maintained dry.

Violation: The deck drain was soiled under the left dipper well.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Violation: There were exposed wires and cables on the unfinished deckhead. Exposed lagging was on the far bulkhead. The bulkhead-deck juncture was not coved.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Site: Housekeeping-Spa
Violation: There was no means to exit the toilet rooms in the sauna/steam changing rooms without touching the door handle with bare hands.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.

Violation: There were no sanitary wipes in the children's toilet room.
Recommendation: Ensure that each toilet facility is provided with a supply of toilet tissue, disposable gloves, and sanitary wipes.
