    Cruise Ship's Corrective Action Statement

Item No.: 08
Site: Galley-Port Coffee Station
Violation: The potable water line for the juice dispenser had a non-continuous pressure device installed. A valve was located downstream from its location.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Violation: The spray hose did not have a backflow prevention device installed on the potable water line. The hose no longer had an air gap.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Potable Water-Cross-Connection Control Program
Violation: The air gaps for the mineralizer backwash discharge lines and the technical water fill lines were not listed in the cross-connection control program.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-AC Fan Room 211
Violation: The potable water faucet on the utility sink was threaded for a hose connection but there was no backflow prevention device.
Recommendation: The vessel shall provide a comprehensive cross-connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets.

Site: Potable Water-AC Fan Room 211
Violation: The potable water lines to the utility sink were not striped blue.
Recommendation: Ensure that potable water piping and fittings are painted or stripped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Site: Whirlpool/Spa-Anti-Entrapment
Violation: There was no documentation that the drain covers on the suction fittings for the forward and aft whirlpools were anti-entrapment.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Site: Galley-Port Water Station
Violation: The drinking fountain had a cup filling spout attached. When asked, staff stated that the spout was used to fill cups for employees working in the galley.
Recommendation: Ensure that employees eat, drink, or use any form of tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils, and linens; unwrapped single-service and single-use articles; or other items needing protection can not result.

Site: Buffet-Trident Grill
Violation: Time only as a public health control was being used for the lunch and dinner service. Hours of operation exceeded four hours. Per staff, food was placed out for service and 3.5 hours later all food on the line was discarded. New food was placed on the line and all food discarded at the end of service which was within four hours. This was not reflected on the times as a public health control plan.
Recommendation: Ensure that the time only as a public health control plan reflects the operations in the Trident Grill.

Site: Buffet-Officer and Crew Mess
Violation: Plastic bins, with covers, of crackers and breads were out for service and not under a sneeze guard.
Recommendation: Ensure that food on display is protected from contamination by the use of packaging; counter, service line, or salad bar food guards; display cases; or other effective means.

Site: Galley-Pastry
Violation: The table top cutting boards in the baking area had rough edges.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: There were slotted fasteners inside the food contact area of the potato peelers.
Recommendation: Replace slotted fasteners with smooth, low profile non-corroding fasteners.

Recommendation: Replace the gaskets.

Violation: The coating on the dough hook for the floor mounted mixer was chipped and peeling.
Recommendation: Ensure that materials used in the construction of multiuse utensils and food-contact surfaces of equipment are: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Violation: The pipe insulation in the technical compartment under the chocolate warmer was torn and split.
Recommendation: Replace the insulation.

Violation: The corners of the right oven below the flat top grill were soiled with grease and food.
Recommendation: Ensure that food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Violation: The seams around the worn gaskets of the potato peelers were soiled with dried food.
Recommendation: Ensure that food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Violation: The water lines inside the juice machine were soiled on the outside of the tubing.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: Clean dishware was stored on a table underneath a detergent dispenser.
Recommendation: Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 150 mm (6 inches) above the deck.

Violation: The deck/bulkhead juncture in the baking area was not coved.
Recommendation: Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters).

Violation: The technical space for undercounter refrigeration unit 4811.5413 had a hole to the bulkhead.
Recommendation: Close the hole.

Violation: There was missing and recessed grout in the food preparation areas, especially around the scuppers and the stainless steel/tile interfaces.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Site: Integrated Pest Management-
Violation: The integrated pest management plan was last revised in 2004. Per staff, a consultant reviewed the plan in May of 2008. No documentation was available for this review.
Recommendation: Ensure that the vessel's Integrated Pest Management Plan is evaluated for effectiveness every 2 years or whenever there is a significant change in the vessel's structure such as a renovation or operation.
