First-Listed External Cause of Injury In the National Hospital Ambulatory Medical Care Survey, this is the first-listed external cause of injury coded from the Patient Record Form (PRF). Up to three causes of injury can be reported on the PRF. Injuries are coded by NCHS to the International Classification of Diseases, Ninth Revision, Clinical Modification Supplementary Classification of External Causes of Injury and Poisoning.
