Planning for the future

The law says your team has to start getting you ready for the future when you turn 14 by writing an Individualized Transition Plan or ITP. Your ITP will be a part of your IEP and will list the services you will need after high school – whether you continue on to college, get a job, or spend time at home. Your team includes your teacher, counselor, parents, doctor, helpful state agencies, and you.

Read over your Individualized Education Program (IEP)

Every student has an Individualized Education Program or IEP. The first step is to ask your teachers, counselors, and parents to hold a meeting to talk about your IEP and your goals for the future – whether you want to go to college, find a job, live on your own, or all of these things! Your teacher or counselor may have already asked you to be a part of a meeting like this. If not, it is always okay to ask.
