North Country Health Consortium, Inc.

North Country Health Consortium (NCHC) was created in 1997 to foster collaboration in building a regional health care system for Northern New Hampshire. Its current members include four Critical Access Acute Care Hospital Systems, an Acute Care Rural Hospital System, two Federally Qualified Community Health Centers, a Mental Health and Developmental Service System, two home care agencies, and a community action program.

Key NCHC services and programs include:

• North Country Health Information Network (NCHIN);
• Northern New Hampshire Area Health Education Center (AHEC);
• Community Substance Abuse Program;
• Community Care Coordination;
• Rural Transportation Network; and
• North Country Mobile Dental Service.
