1. You must complete form ITA-2064, "DOC Application for Transit Benefit", which can be located by clicking on the icon "ITA Forms" available on ITA networks. You can also pick up a copy of the application in Room 4001.

2. You must complete form ITA-2065, "Government Travel Card (Individually Billed Account) Set Up Form", which can be found by clicking on the icon "ITA Forms" available on ITA networks. You can also pick up a copy of the application package in Room 4001.

3. A traveler can obtain the status of his/her travel voucher by calling 303-969-7780 ext. 2565. Please wait two week after the travel voucher is submitted to DOI/NBC.

4. If you provided all requested material without withholding or redacting any documents, the file must be kept for 2 years after the date of your reply. It can then be destroyed. If documents were withheld or redacted, the file must be kept for 6 years after the date of reply. If the FOIA request is appealed, the file must be maintained for 6 years after the final determination by the agency, or maintained for 3 years after final adjudication by the courts, whichever is later.

5. Yes. All collections of data from 10 or more members of the public are subject to the Paperwork Reduction Act.

6. Yes. General guidelines for webforms are as follow:

  • OMB-approved information collection forms do not require any additional clearance when placed on a website but must display the OMB Control Number and Expiration Date, Burden and a Privacy Act Statement.
  • Website forms with an undifferentiated comment box that merely ask for general comments (e.g., Tell us what you think), name, address and/or email address do not require OMB-approval.
  • Website forms that request respondents to tell the government about anything specific in addition to providing general comments, name, address and/or email address require OMB-approval.

    7. Such requests must be submitted to the ITA Secretariat in writing--either through e-mail or on the correct form for submission of the requests. The ITA Secretariat will notify the correspondence contact for your unit when the request is approved or disapproved by the Department's Executive Secretariat.

    9.     10. Dial the MCI country-specific toll-free access number of the country you are calling from. Follow the voice instructions or wait for an MCI operator to complete your call. The country toll-free access numbers are in the WorldPhone Guide provided to each FTS2001 MCI cardholder when they receive their calling card.
