The Office of Personnel Management (OPM) has revised Standard Form (SF) 2800, Application for Death Benefits (Civil Service Retirement System), to bring it up-to-date and to ask whether a court has awarded a survivor benefit to any former spouse. SF 2800A, Documentation and Elections in Support of Application for Death Benefits (Civil Service Retirement System) When Deceased Was an Employee at the Time of Death, has been revised to bring it up-to-date. SF 2800-1, Applying for Death Benefits Under the Civil Service Retirement System, has been revised to bring it up-to-date and to give more detailed information about children’s benefits.

The only acceptable versions of these forms are as stated above. Please destroy any prior versions you have in stock.

These forms can be obtained from the Office of Personnel Management (OPM) web site at www.opm.gov/forms/html/sf.asp. They are no longer printed and stocked by the General Services Administration.
