Safety Hazard Information Bulletin on the Use of
Merrill-Chinbro Beam Clamps |
The New York Regional Office has brought to our attention a serious hazard
existing with the use of certain lifting clamps manufactured by the Campbell
Chain Division of Cooper Tools. An accident investigation has revealed that
the cause of the accident was the failure of a Merrill-Chinbro beam clamp.
The manufacturer discovered a potentially serious defect involving possible
pin failures in its Merrill-Chinbro beam clamps which would result in the
Clamps dropping their loads. The defective clamps are identified as
Merrill-Chinbro models F-15, NS-15, F-25, NS-25, F-35, and NS-35. Campbell
Chain has removed the clamps from its product line and has advised previous
purchasers to discontinue the use of these models immediately. (See
attachment.) Since some of the defective clamps may still be in use,
compliance officers and consultants should be aware of the hazard associated
with use of the above models.
Please distribute this bulletin to all Area Offices, State Plan States and
Consultation Projects.

Dear Merrill Customer:
Campbell Chain, a unit of Cooper Tools has discovered a potential danger
which could result from the use of Merrill Chinbro Clamps Model numbers F15,
NS15, F25, NS25, F35, NS35. As a result, we have released the attached
recall announcement and are making all reasonable efforts to notify each user
of the recall.
Our sales records show that your company has purchased one or more of the
clamps involved in this recall. Could you please supply us with the names,
addresses, and phone numbers of customers who have purchased these clamps
from you so that we may contact them and arrange for the return of the
clamps.

Campbell Chain, a unit of CooperTools has discovered a potential danger
which could result from the use of MERRILL Chinbro Clamp Models number F-15,
NS-15, F-25, NS-25, F-35, and NS-35. Continued use of these clamps may
result in the clamp dropping its load. SERIOUS PERSONAL INJURY, PROPERTY
DAMAGE OR BOTH MAY OCCUR FROM THE USE OF THESE CLAMPS. Please discontinue
the use of these models immediately.
Please call collect our Merrill Safety Engineer at 717-755-2933 to arrange
for the recall of all subject clamps. The expense for freight, repair or
fair value (if unrepairable) will be covered by Campbell Chain. All written
communications regarding this recall may be directed to the address below.
