Complete An Account Request Form To establish a NAS account, print a copy of the two-page Account Request Form. Read all information on this form before beginning. Direct any questions to the NAS User Interface Manager at nop@nas.nasa.gov. When all entries are complete, sign the Requestor signature block, and pass the form to your principal investigator for authorization. If you are a principal investigator applying for an account, you must sign in both locations. 2.
