For previously approved studies in which the scope and/or purpose has not changed, submit the following:

• Letter of Request:
  • This letter must indicate the Data Use Agreement number, study name, and data set(s) you need to acquire from the Centers for Medicare & Medicaid Services (CMS) to continue your study. Also include a copy of the signed and approved Privacy Board Summary Sheet.

Note: If you need to change the purpose and/or scope of a previously approved study by the Privacy Board, you must resubmit your study as a new request.

Send your Letter of Request along with the copy of the Privacy Board Summary Sheet to the following mailing address:
