Specimen Request Information

Requests for blood specimens from the NHLBI Blood Specimen Repository are made directly to the Transfusion Medicine and Cellular Therapeutics Branch of the National Heart, Lung, and Blood Institute. Please read these instructions carefully before completing your request forms.

Before mailing your Request Forms, make sure that you:

1. Sign the Request Forms, initialing by hand where indicated.

2. Check that your request documents contain original signatures.

The NHLBI Blood Specimen Repository cannot accept request forms by fax. You must mail your requests with the original signatures to the address below.

3. Complete the Standard Indemnification Agreement, the State Institution Compliance Agreement, or the Waiver of Indemnification Agreement . Please determine which of these forms is appropriate for your institution and submit only one of them. US Government employees are not required to submit an Indemnification Agreement.

4. Obtain the proper signatures. Where the application forms specify Officer of Institution or Company, signatures must be obtained from an official capable of legally binding the Institution (e.g. president, vice-president, dean, or provost, but not a department chairman). As a requestor, you must be a Principal Investigator, Laboratory Director, or equivalent (public or academic institution), or a Director of Research or equivalent (private or for-profit institution).

5. Attach a Biographical Sketch that includes a listing of five recent, representative publications, or a brief curriculum vitae. A Biographical Sketch from a recent NIH grant proposal may be submitted instead.

6. Provide a FedEx (or other shipping company) account number. Your carrier must be able to ship packages containing dry ice and biohazardous materials, if appropriate.

7. Send an electronic copy of the proposed research protocol to the Project Officer.
