The Academy of Health Information Professionals

Forms for the Academy Portfolio

The level of desired academy membership and type of application (initial or renewal) determines the forms needed for the portfolio. Consult the Application Process or the Renewal Process for details. 

The following forms are available as PDF (Portable Document Format) versions which can be printed out and completed manually. You will need a copy of the free Adobe Acrobat Reader to view the PDF files. 

Some forms are also available as RTF (Rich Text Format) files which can be opened in most word processing programs. These forms may be completed electronically, printed, and submitted. For a hard copy of any of these forms, contact MLA headquarters.
