10-year Trends in Penicillin- and Erythromycin-resistant S. pneumoniae for 5 European Countries and the USA: The Alexander Project.

BACKGROUND: The Alexander Project is an ongoing, prospective surveillance study. METHODS: Isolates were collected from centers in France, Germany, Italy, Spain, UK and USA annually in 1992-2001. MICs and resistance (R) to penicillin (PEN), erythromycin (ERY) and cotrimoxazole (COT) were determined according to NCCLS methodology, risk ratios (RR) calculated for the cumulative resistance prevalence for 1992 and 2001 and p values obtained using the chi-square test. RESULTS: See table. CONCLUSIONS: Resistance prevalence varied greatly across these countries. The average increase over 10 years in Europe was 8.1% for PEN-R and 19.6% for ERY-R. The odds of an increase in resistance was 2.43X higher for ERY than PEN in Europe (95% CI 1.82, 3.32; p <0.0001), 1.22X higher in USA (p = 0.7). In Europe and USA, 43.6% and 72% of ERY-R strains, respectively, were cross-resistant to COT in 2001. The increases in the prevalence of ERY-R and cross-resistance with COT suggest that the role of empiric therapy with certain classes of drug (e.g. macrolides) may need to be re-evaluated in these countries.
