Hurricane Katrina Relief Efforts – Urgent Request

Attention: Mental Health and Substance Abuse Agencies

The following HHS Critical Infrastructure Data Request is for use by substance abuse and mental health facilities in the States that have a disaster or emergency declaration resulting from Hurricane Katrina.

As of 9/20/05, those States are: AL, AR, AZ, CA, CO, CT, Washington (DC), FL, GA, IA, ID, IL, IN, KS, KY, LA, MA, MD, MI, MN, MO, MS, MT, NC, ND, NE, NM, NV, OH, OK, OR, PA, SC, SD, TN, TX, UT, VA, WA, WI, WV. All substance abuse and mental health facilities in those States are encouraged to report this voluntary information to the best of their ability.
