The Non-Emergency Medical Transportation Program

Effective
November 1, 2006, LogistiCare Solutions, LLC began managing the Non-Emergency Medical
Transportation Program for the Division of Medicaid. LogistiCare will arrange transportation
to take you to services covered by Medicaid if you have no other means of transportation.
Transportation arrangements to routine medical appointments must be made 72 hours
(3 business days) in advance.

Before you call, please have the following information ready:

- your current Medicaid ID number;
- time and date of your appointment;
- address and phone number of the medical facility; and
- reason for your appointment.
