Quality Control Limits for Mecillinam Disk Susceptibility Testing Against E. coli ATCC 25922 Based on NCCLS Methodology.

Discussion: Mecillinam (MEC) is a unique b-lactam antibiotic classified as an amidinopenicillin. Unlike other b-lactams which bind to penicillin binding protein (PBP) 1 and 3, MEC has an affinity for PBP 2. This characteristic gives MEC selective activity against members of the Enterobacteriaceae such as E. coli, Klebsiella, Enterobacter, Citrobacter. MEC has low activity against Staphylococcus aureus, and Pseudomonas. MEC is approved for use in acute uncomplicated cystitis (AUC). There is a 20 year clinical history in Scandinavia where MEC is recommended as a first-line agent in AUC. Resistance levels have remained steady at 0 to 4%. Objective: There are no disk diffusion quality control (QC) limits for MEC based on NCCLS methodology. We undertook a collaborative, international, multicentre study to define QC limits for MEC 10 microg disk diffusion testing against E. coli ATCC 25922 based on NCCLS methodology. Method: Eight centres tested 2 different manufacturers lots of MEC disks and an ampicillin 10 microg control disk, on 3 different manufacturers lots of Mueller-Hinton Agar for 10 days. E. coli ATCC 25922 was the test strain. Out-of-control results were repeated and only results in-control were analyzed. Statistical analysis was based on the median statistic of Gavan et al. All methods followed established NCCLS guidelines.RESULTS: A total of 480 data points were produced and analyzed. Based on the median statistic of Gavan et al, control limits for MEC against E. coli ATCC 25922 would result in a span of 3 mm on either side of the all-laboratory median of 27 mm. This produced a range of 24 to 30 mm that included 97.5% of all data points. Summary: We propose a disk diffusion QC limits range of 24 to 30 mm for MEC 10 microg against E. coli ATCC 25922.KEYWORDS: Disk diffusion; Mecillinam; Urinary tract infection
