Quality assurance program for anti-HIV-1 detection among 165 blood banks laboratories from the nation-wide Mexican health system.

OBJECTIVE: To verify the proficiency of 165 laboratories in blood centers and blood banks in charge of anti-HIV-1 screening of blood units, to assure the safety of blood supply. METHODS: Laboratories participating in the CNTS Model Performance Evaluation Program since July 1989 furnished EIA and/or passive aglutination testing. An evaluation panel was sent them each 6 months. Each panel consisted of 7 individual samples including negative, positive, and weak positive, adequated to the type of screening tests used at each laboratory. Sample reactivity was based on composite EIA and western blot testing at CNTS. Records of results were obtained by the CNTS for analysis. RESULTS: Laboratories reported 8 (0.06%) false positive for negative samples, compared with 6 (1.7%) false negative for positive samples and 44 (8%) false negative for the weak positives. Intra-shipment reproducibility was 15% for positive samples and 30% for weak positives. False positives were observed mainly with serodia test (5/8). False positives observed in ELISA were parallel to 2 false negatives for positive samples in the same 2 big laboratories in Mexico City. CONCLUSION: Most laboratories experienced no difficulty in testing the negative and positive samples, but clerical errors were detected in 2 big laboratories processing large amounts of samples. Some technical difficulty was observed in the weak positives in well set labs. A close contact and technical help is required to achieve best performance.
