Note:
Starting with reporting on 2002 incidents, PHMSA is using a new
definition of "serious incident". In the
"Serious Incidents (New Definition)"
report, years prior to 2002 are also based on this new definition.
The "Serious Incidents (Original Definition)"
report provides the 1992-2001 serious incidents based on the original definition.
Incident Report Form 5800.1 Data
The following files contain the Incident Report Form 5800.1 data submitted
under the requirements of Title 49 CFR 171.15 and 171.16 for the calendar year
2000.
The incident data are contained within three files.
These files are provided as self extracting executables (*.exe) for IBM compatible
machines and as zip files (*.zip) for MacIntosh machines.
NOTE: IBM compatible machines can read both *.exe and *.zip files.

The Readme.pdf (7KB) and Userguide.pdf (68KB)
contains instructions for importing the data into a table.
