Forms Note: You may type directly into the form on your computer in the Application Form (PTO 158)*, Undertaking Form (PTO 275)*, and the Credit Card Authorization form (FORM PTO 2038). After typing the information into the form on your computer, print the form(s) locally at your printer, sign the forms and submit in accordance with the instructions in the General Requirements Bulletin.
