Subject: Federal Employees Health Benefits (FEHB) Program: Opportunity for Enrollees of American Healthcare Providers, Inc. (American HMO) Enrollment Code AC, to Change Enrollment


American Healthcare Providers, Inc. (AHCP), enrollment code AC, in the states of Illinois and Indiana will be liquidated and will no longer participate in the FEHB Program after July 1, 2000.

We are authorizing an opportunity for enrollees of AHCP to change their enrollment. The opportunity to change enrollment will begin immediately, and continue through June 30, 2000. All enrollees must change their enrollment to another participating plan in order to continue to receive benefits under the FEHB Program for the remainder of the 2000 contract year. The effective date of the new coverage may be made retroactive to the pay period beginning on or after May 1, 2000 but no later than July 2, 2000. Enrollees who do not change their enrollment by June 30, 2000 will not have benefits after July 1, 2000.

A special notice was sent to enrollees advising them to make a change to a new plan. We have attached a copy of that letter for your information.
