Home and Community Living Supports

Basic Services

Congregate Meals:
The congregate meal program provides a nutritious meal for older Arizonans in senior centers across the state. Many of the centers also have a wide variety of leisure time activities. 

Home Delivered Meals:
This service provides a nutritious meal delivered to the home.

Home Health Aide:
This service provides health maintenance, treatment, monitoring, and support for activities of daily living.

Home Nursing:
This service provides skilled nursing services at home for people who need help with health care such as taking medications, changing dressings, catheter care or other skilled nursing services.

Housekeeper/Chore:
This service provides assistance for routine household maintenance in the person's home.

Personal Care:
This service provides assistance to meet essential personal physical needs.

Respite:
This service is intended to relieve a caregiver of their caregiving duties for a short period of time. Some communities offer volunteer or paid respite care services which provide short-term, temporary care for an impaired older person to relieve the family members who provide daily care to their relative.

Transportation:
This services is intended to help older persons to get around the community. A number of communities offer door-to-door transportation services for older persons such as vans or mini-buses which accommodate wheelchairs, walkers, and other devices. Transportation may be provided to and from the doctor's office or other medical services, community facilities and other services. Help is also available in some communities in the form of escort services and shopping assistance.


Additional Services

Adult Day Care:
Adult day care services are available in some communities. This type of service provides social and some rehabilitative activities for the frail older person or disabled adult during the day in a community facility.

Home Repair and Adaptation:
Limited home improvement services are available to older persons who meet income eligibility guidelines. Such activities include roofing, ramps, and insulation.


Eligibility

Eligibility for Services:
To be eligible for services, you must be at least 60 years of age or 18 years of age with a disability.

Case Management:
Case management is the process by which need and eligibility for services are determined. This is a prerequisite for most services.
