Standard Operating Procedure Table of Contents

Purpose
To prevent NIAID from making an award before documentation is in place
for research
misconduct.
Procedure
Institutions that apply for or receive Public
Health Service funding -- except for conference
grants -- are required
to establish and maintain a misconduct in science assurance with the Office
of Research Integrity (ORI).
This assurance certifies that
an institution has a process for responding
to allegations of research
misconduct, which meets the requirements of federal regulation 42
CFR Part 93.
To establish an assurance,
the institutional
business official signs the SF
424 (Cover Page) form (for an electronic
application) or the face
page (for a paper application).
The grants management specialist forwards the cover page to
ORI, who will establish a record for that institution and send a welcome
letter.
Institutions must maintain an assurance by
filing an
Annual Report on Possible
Research Misconduct, Form 6349, through
ORI's Annual Report System.
ORI will impose
a bar to award if it does not receive this annual report.
In addition to an assurance, an institution must provide its policies
and procedures to ORI upon request.
Grants Management Specialists
A "red bar" in IMPAC means that
an institution has not submitted (a) its annual report, or (b) its
misconduct policy upon ORI's request. Red bar
applications cannot be awarded.
To lift a red bar:
- Email Contact
for NIAID Staff, ORI's
assurance program manager.
Include the institutional profile (IPF) number, name of institution,
and grant number.
- ORI will take steps to obtain necessary documents
and will remove the bar
from IMPAC once the documents are received and approved.
A "yellow warning," most often used for new institutions,
means that an assurance has not been established
with ORI. Applications
with a yellow warning should still be awarded.
To lift a yellow
warning:
- Email Contact
for NIAID Staff, attaching
a PDF of the face page or SF
424 (Cover Page) form. If you're
unable to email either document, fax it to ORI with a cover letter
at 301/594-0042.
- Once ORI receives the appropriate document,
it will establish an assurance and update the institution's IMPAC
record.
