Deaf or Hard of Hearing Placard

A Placard is available from the Bureau of Motor Vehicles to identify motorists as Deaf or hard of hearing. This Placard is available only to deaf/hard of hearing motorists. It helps law enforcement officials recognize deaf or hard of hearing drivers in order to make any necessary communication accommodations.

The driver should place the placard in a conspicuous location on the dash or visor of the automobile, as close to them as possible, without obstructing their view.

To apply for the placard, please fill in the information on the top half of the Division of Deafness application and have a licensed physician, Ear Nose & Throat specialist, or audiologist complete the bottom half. We will also accept a diploma from a school for the deaf as proof of deafness. You can obtain the DoD application form by contacting either:

After we receive your application for approval, we will send you a letter of certification and your Motor Vehicle Placard application. You will need to take the letter of certification (that we send you) and the placard application to your local Motor Vehicle office to get your placard.
