The Minnesota Department of Human Services supports quality care and services for Minnesotans with physical disabilities, chronic health conditions, developmental disabilities, mental illness, chemical dependency and HIV/AIDS so they can live as independently as possible.

HIV/AIDS
In Minnesota, about 5,950 people are known to be infected with HIV. The HIV/AIDS Division of the Minnesota Department of Human Services manages publicly-funded programs that support people living with HIV/AIDS to access life-extending medication therapy and medical care. The HIV/AIDS Division Services, commonly referred to as Program HH Services, provide access to HIV medications, insurance benefits, dental services, mental health services and nutrition services for HIV-positive persons who meet program eligibility guidelines.

Six
month update returning August 1, 2008
All clients using Program HH services must complete and submit an annual application to remain eligible for the program. As part of our federal obligations, we will also begin collecting a six month update. This six month update must be filled out and returned similar to the annual application. This six month update is federally mandated and enables us to continue to receive Ryan White funding. Ryan White money is the single biggest source of funding for Program HH and is essential for us to continue offering our programs and services.

The release of the 6 month update is August 1, 2008. The update form will be mailed to you. When you receive it please fill it out and return it to us. We will begin mailing out six month update forms on July 1 as part of a phased in process. Not everyone will receive the update all at once. You will receive the update based on when your annual application is due. If you have not yet reached the 6 month mark from the time your annual reapplication is due by August 1 then you will receive the update form in 2008. If you have already passed the 6 month point for your annual reapplication, you will not receive the update form until 2009.

If you have any questions about annual reapplications or the new 6 month update form please contact the program line at 651.431.2414
