Contractor Clearance Forms

Below are the necessary forms for contract personnel to complete before starting to work at SSA/OIG:

1.  OF-306 Declaration for Federal Employment

2.  GSA-176 Statement of Personal History

3.  Fair Credit Reporting Act

4. Two (2) completed forms FD-258 (Fingerprint Charts). These forms can be obtained from the SSA/OIG Project Manager. The contractor will absorb the costs for obtaining fingerprints. Fingerprints may be obtained at a local Police Station.

Upon completion of forms 1-3, please remit to:

4.  Confidentiality and Non-Disclosure Agreement

5.  SSA-4395 Application for Access to SSA Headquarters Facilities

Upon completion of forms 4 + 5, please remit to the appropriate SSA/OIG Project Manager.
