Obtaining a State Application (STD. 678)
You may obtain an official State Application (STD. 678) from the Department of Rehabilitation at 721 Capitol Mall, or the State Personnel Board, 801 Capitol Mall, both in Sacramento, CA or any local California Employment Development Department office. Out of State applicants may get an official state application form by sending a self-addressed stamped envelope to:
