Completing the Application:
- You may want to print this
instruction page for your use.
- You will need to save the
application to your computer, complete the fillable form, print, and
mail as directed below.
- You should also print copies of the
Reference Form (provided at the end of the application) to
give to those providing references for you.
- It is the student's responsibility
to ensure that reference forms are received by the application
deadline.
Required Items:
- Completed
application form.
- Two (2)
references. (Have your referees submit these on your behalf
using the Reference Form provided in the application materials.)
- Official copies
of your transcripts for all colleges or universities you have
attended including grades for any courses taken during fall term
2005. (Have
the schools mail the transcripts directly to ORISE to the address provided below.)
Deadlines:
- April 22, 2006:
Application must be received by this date.
- April 22, 2006:
Two Reference Forms must be received
by this date.
- April 22, 2006:
Official Transcripts from all colleges or universities you have
attended must be received by this date.
