REACH 2010 Coalition
Reducing diabetes health disparities experienced by communities of color
Diabetes in King County, April 2007
The number of people in King County with diabetes doubled in the past decade. Such a rapid increase in the occurrence of a chronic illness is very rare. Diabetes now affects 84,000 adults in King County or 5.8% of the adult population.

REACH mission
The mission of the REACH 2010 Coalition is to reduce diabetes health disparities experienced by communities of color. Through strong partnerships, the Coalition supports the empowerment of individuals, families, and communities, and creates sustainable long-term approaches to prevention and control of diabetes by utilizing all appropriate community resources in King County.
REACH overview
The REACH Coalition, which includes Public Health-Seattle & King County, works cooperatively toward the goal of eliminating diabetes-related health disparities among African American, Asian American/Pacific Islander American, and Hispanic/Latino communities in King County. This effort is funded by the Centers for Disease Control and Prevention.
REACH goals
- Increase opportunities for diabetes education and self-care
- Increase personal awareness of potential risk for diabetes
- Increase community awareness of diabetes
- Increase community support for people with diabetes
Community Action Plan includes:
- Support groups
- Peer education
- Education classes
- Self-management
- Registry enhancement
- Restaurant and grocery store campaigns
- Faith-based education
- Worksite education
- Pharmacy education
- Media
- Information to the community
Events:
REACH Coalition meetings are held from 3:00 - 5:00 PM on the fourth Thursday bimonthly starting January 26, 2006. Please contact a REACH referral site for the location of each Coalition meeting.
