Some Pharmaceutical Companies offer assistance programs for the drugs they manufacture. To see if any programs are available for the drugs you are taking, please select the letter in the list below to see if your drug is on the list. Eligibility Criteria | Patients with prescription coverage, including those enrolled in a Medicare Prescription Drug Plan (Part D), who are experiencing financial or medical hardship may apply for "Hardship Exception" in the Connection to Care program.
A patient must first apply to Connection to Care. If a patient meets the income requirement for Connection to Care (income at or below $19,600 per year if single or $26,400 per year if married), but has insurance coverage for prescription medicines, they will be sent a "Hardship Exception Request Form."
The patient and their physician will need to complete and sign the form and return it to Connection to Care.
Each Hardship Exception Request will be reviewed on a case-by-case basis. |
Benefits/Assistance | If the Hardship Exception Request is approved, the patient will receive their Pfizer medicines at no charge. The 90-day supply of medicine is shipped to the physician's office for pick-up by the patient. |
