FOIA Request Form

This form can only be used to request information from the Federal Law Enforcement Training Center (FLETC) under the FOIA. We will not accept PA requests using this form. In addition, if the requested records contain personal information, we may ask you to establish your identity by requesting the information in writing and signing the request by hand.

This form collects information that is protected under the Privacy Act (i.e., home address and home phone). Please read the Privacy Act Statement below.

Include such items as subject, contract number, time frame, author, and recipient. Also include the type of record i.e., memorandum, letter, report, form, etc. Remember, the FOIA is not a forum for answering questions.
