To the extent practical, all instructions will be implemented 90 days after they are included in the Quarterly Provider Update. However, some instructions and program changes have external dependencies that will necessitate a mid-quarter publication due to their significance to the provider community.  A list of these instructions/changes are identified in the "Downloads" section below.
