What is CCPED?
CCPED is a Medicaid Waiver program that provides a range of services to enable an individual, at risk of placement in a nursing facility and who meets income and resource requirements, to remain in his/her community home. By providing a uniquely designed package of supports for the individual, CCPED delays or prevents placement in a nursing facility.
What services may be provided?
Based on the results of a clinical assessment, a Plan of Care (POC) is developed collaboratively by the participant and his/her Care Manager. All CCPED participants receive Care Management services. In addition, the POC specifies other services to be delivered, which may include:
- Respite Care
- Homemaker Services
- Social Day Care
- Home Health Services
- Adult Day Health Services
- Non-emergency Medical Transportation
- Prescribed drugs
CCPED consumers do not receive other Medicaid services.
Are there limits to CCPED services?
CCPED is available statewide. Program costs are restricted by limits on the number of community care “slots” and per-person costs. A care manager helps the consumer develop a service plan, arranges for services with provider agencies, and monitors services and costs.
Who provides these services?
All providers of services meet required licensing and/or certification standards and adhere to other local, State and Federal standards. All service providers are approved and enrolled as Medicaid providers.
Who is eligible to receive CCPED services?
- Individuals who are 65 or over and eligible for Medicare or have other health insurance that includes hospital and physician coverage; or
- Individuals who are under 65 and determined disabled by the Social Security Administration and eligible for Medicare, or determined disabled by the New Jersey Department of Human Services, Division of Medical Assistance and Health Services, Disability Review Section, and have other health insurance including hospital and physician coverage; and
- The individual has been assessed by the staff of the New Jersey Department of Health and Senior Services, Regional Office of Community Choice Options, and found to be in need of nursing facility level of care; and
- The individual's gross monthly income does not exceed $1,911 per month in 2008 and his or her resources are at or below $2,000 (or $3,000 for a couple); or
- The individual is currently ineligible for SSI, New Jersey Care, or Medicaid in the community.
- A spouse's income is not considered in the determination of eligibility, but the spouse's resources are considered.
Is there a co-pay in CCPED?
There is no co-pay in CCPED.
