Veterinary Biologics Permits

As of February 15, 2007, veterinary biological product permits for research and evaluation, and transit shipment became available through the United States Department of Agriculture (USDA), Animal and Plant Health Inspection Service ePermits system. ePermits allows you to submit permit applications, track applications, and receive a copy of the permit on-line, or by fax or email.

Alternatively, you may continue to submit applications directly to the Center for Veterinary Biologics (CVB), Riverdale, MD.

Permits are offered through the on-line web-based ePermits system.
Apply on-line (for eAuthenticated users only):

APHIS Form 2005 – Application for United States Veterinary Biological Product Permit to import veterinary biologics for research and evaluation or transit shipment.

Fax or mail your application (for non eAuthenticated users):

APHIS Form 2005 (1.52MB) – This form should only be used if applying for a permit to import animal vaccine or diagnostic test kits.  Permits for other animal products, by-products, or organisms and vectors, apply using VS Form(s) 16-3, 16-7, or 17-129 which can found under "Animal Health Permits".
