Required Forms

For more efficient orientation, all forms may be filled out prior to arrival with the exception of the I-9 form.

Employee Profile Form (pdf)
W-4 Form: Complete Form W-4 so that your employer can withhold the correct Federal income tax from your pay. Because your tax situation may change, you may want to refigure your withholding each year.
Laboratory policy regarding government property (pdf)
List of Acceptable Documents (pdf) (for I.D. and Employment Eligibility (I-9): Required documents at the time of hire.
Payroll Direct Deposit (pdf): This form will be submitted to payroll to allow for direct payroll deposits to your checking and/or savings accounts.

QNSP (pdf): To be completed if your job requires a Q clearance. Check with your new manager before filling out the QNSP paperwork. The Laboratory has a limited number of allocations for Q clearances. New allocations come out once a year. Ensure you have an allocation before submitting your paperwork otherwise it will not be processed. Once you receive notification that there is an allocation, submit it to either HR or your manager and ask that they arrange a meeting with the Security office during your house-hunting trip. Unfortunately, the Lab will not pay for two trips.
