Project Hanford Lessons Learned

Opportunity missed (22K)
Excerpt from Lessons Learned videotape.

Welcome to the Project Hanford Lessons Learned Web site. One of the primary goals of the Department of Energy Lessons Learned process is to connect DOE Sites through shared information so we can all benefit from each other's innovations and prevent repeating unfortunate events. Here you can find indexes of Project Hanford Lessons Learned, a description of the lessons learned process, reference material, and links to the DOE Lessons Learned database.

Lessons Learned by Year

Most Project Hanford Lessons Learned are in the DOE Lessons Learned database and can easily be found by searching in one or more categories on that page. We recommend you look there first.  You can also find lessons learned from other DOE sites there.

For help finding particular lessons learned here or in the DOE database, contact the Project Hanford Lessons Learned Coordinator by telephone on (509) 372-2166 or by e-mail.

Definition
Lessons Learned:
A "good work practice" or innovative approach that is captured and shared to promote repeat applications or an adverse work practice or experience that is captured and shared to avoid a recurrence

Process Description

The Lessons Learned distribution process starts with the Project Hanford Lessons Learned Coordinator screening various sources of information, including ORPS, DOE publications, and Listserver messages. He then issues applicable lessons to Points of Contact (POCs) in each major organization. Those POCs then further distribute the lessons learned bulletins to appropriate personnel within their organizations.

Priority Descriptors

Each lessons learned is assigned a priority descriptor depending on the severity of the event that generated the lesson. They are generally described by the following criteria:

  • RED/Urgent - Fatality or serious injury, significant environmental release, violations of law with significant penalties, widespread public interest, etc.
  • YELLOW/Caution - Injury, temporary disability, uncontrolled release of moderate size, violations of law with minor penalties, significant non-compliance with DOE orders or regulations, a potential event that may have affected the public had it occurred.
  • BLUE/Information - Information to protect public and worker safety, environmental quality, or to improve public relations.
  • GREEN/Good Work Practice - Any action or activity that improves operations, compliance, efficiency, performance, or promotes benefits to the public.

Detailed criteria for determining the appropriate codes are contained in Table A-1 of DOE-STD-7501-99, reproduced locally.
