To assist you in finding the appropriate permit application forms, we
have separated
all of the permit application forms into groups. The groups are logical, similar program type groupings.
Each group contains all the forms in that group individually. In addition, each group contains a .zip file
which contains all the individual forms in that category. All forms are available individually or in the master zip file as .pdf
or .xls files.
NOTE: To DOWNLOAD AND OPEN these Acrobat pdf forms you MUST use Adobe
Acrobat or Adobe Acrobat Reader 5.0 or later (see link below).
If you wish to
save the completed form on your local storage media you MUST use the full
version of Adobe Acrobat 7.0 or later.
However, if the Acrobat AutoComplete feature is turned on, any data
that you type into a form field will be available the next time you use the
form. THIS DATA IS ONLY AVAILABLE ON YOUR LOCAL PC; IS IT NOT "SAVED"
with the form.

Attachment A Designation of Agent for Service of Process and Consent to
California Jurisdiction
- to be used in conjunction with Form 2007-1
for entities
not domiciled within the State of California and who are
not RECLAIM
Facility Permit holders.
Note:
Attachment A includes requirement for a certification, contract or other
documentation provided by the designated Agent for Service of Process. Such
documentation shall include:
A. The agent is
licensed Agent for Service of Process in the State of California,
B. The appointing entity (i.e. RTC trader), the start and end date of the
appointment,
If the
appointment is LESS THAN 4 years from the date of receipt, then:
C. Statement that
the appointing entity has requested appointment of 4 or more years, and
D. Maximum term of appointment/contract that the Agent for Service of
Process would accept (e.g. annual, bi-annual).

Supplemental Application Forms (400-E-XX):
In addition to the Basic Forms,
applicants may also be required to submit completed equipment-specific
supplemental forms with the
appropriate fee when applying for a Permit to Construct or Permit to Operate.
Forms 400-E-xx request specific data relating to the facility and its equipment,
process, operating conditions, and emissions. Please note that these forms are prepared for general equipment types. Other
information, such as air quality modeling and risk assessment, may be required in special
cases.
Form 400-E-GI (pdf 71 kb) must be submitted if
no other Form 400-E-xx applies to your specific equipment or process.
