Paper log sheets and spreadsheets are available to help funded partners track completed activities for reporting in the Indicators for School Health Programs. Log sheets and spreadsheets can be modified to suit funded partners’ particular needs. These tools can be accessed from the Data Collection and Submission Resources pages. Funded partners who need assistance with the Indicators or paper logs should contact the Evaluation Research Team at ert@cdc.gov.
