New U.S. Department of State Forms for the Overseas Medical Examination of Immigrants and Refugees
The obsolete Optional Form-157 (OF-157) has been replaced with the new DS-2053 and associated worksheets (DS-3024, DS-3025, and DS-3026). They provide information for the resettling health departments and the Centers for Disease Control and Prevention about the immigrant or refugee. See the detailed instructions on completing the forms. (These instructions are also available in printable PDF format. [ 79 KB, 18 pages]
