* ≥4 doses of diphtheria, tetanus toxoid, and any acellular pertussis vaccine which can include diphtheria and tetanus toxoid vaccine or diphtheria, tetanus toxoid, and pertussis vaccine [DTaP], ≥3 doses of poliovirus vaccine; ≥1 doses of measles, mumps, and rubella vaccine [MMR]; ≥3 doses of Haemophilus influenzae type b [Hib] vaccine; ≥3 doses of hepatitis B vaccine [HepB]; and ≥1 doses of varicella vaccine [VAR])

Children in the 2003 NIS were born between January 2000 and July 2002.
Children in the 2004 NIS were born between January 2001 and July 2003.
Children in the 2005 NIS were born between February 2002 and July 2004.
Children in the 2006 NIS were born between January 2003 and June 2005.
(2006 NIS estimates are based on the NIS dataset which was re-released
on February 25, 2008 after correcting for Hispanic overcount in 9 states)
Children in the 2007 NIS were born between January 2004 and July 2006.

§ Poverty status was based on 2006 U.S. Census poverty thresholds.
