Introduction |
The Application for a Social Security Card (SS-5) can be used by anyone who has never been issued a card, needs a replacement card or has changed his or her name. THIS SERVICE IS FREE. But before you go to the form, please read our new Fact Sheet, because recent changes in law have affected the rules and the documents required for assigning Social Security numbers and issuing cards. |

How to Obtain the Form |
Below you will find the SS-5 Form in Portable Document Format (PDF). To print the PDF version, you will need the Adobe Acrobat reader software. If you do not already have this special software, see our page on downloading and printing PDF documents. NOTE: if you live abroad, and cannot obtain 8.5 x 11 paper, the use of A4 size paper (8.25 x 11.7) is the only acceptable alternative.

Where To Send The Form |
Finally, take or mail the SS-5 to the nearest Social Security office. Be sure to take or mail the originals or certified copies of your supporting documents along with the form. We will return your original documents right away. Depending on your circumstances, you can take or mail your application and evidentiary documents to your local Social Security office. First-time applicants age 12 or older must apply in person at a Social Security office while all others may mail their application materials. In order to enhance public service and strengthen the integrity of the Social Security Number, the Social Security Administration has opened six Social Security Card Centers. People who live or receive mail in Orlando, FL, Brooklyn, NY, Queens, NY, Las Vegas, NV, or the Phoenix, AZ metropolitan areas must apply at one of the following Social Security Card Centers. These offices serve both U.S. citizens and noncitizens applying for new or replacement Social Security cards.
