X. ANNUAL CLUSTER SITE VISITS

Goal: Real time communication, accountability and reporting related to current and postcollaborative
health centers, spread, equity and resource allocation.

Performance Expectations
PCA and CN

  1. Schedule cluster site visit with the BPHC. Attendees for this meeting will include PCA Executive Director,
    Director of CN, HRSA Field Office lead clinician, cluster directors and IS specialists and cluster
    collaborative coordinators.

In support of this National Goal, BPHC plans to:

  1. In collaboration with cluster leads, develop uniform agenda and tools for cluster site visit that address
    status of current and post collaborative health centers, spread, equity and resource allocation.
